Before submitting a nomination, please review the awards criteria document, outlining the requirements and eligibility for each award category. 

To nominate a dispatcher, complete the form below by providing details about the 9-1-1 call, a description of how the dispatcher assisted, and any additional relevant information. You will also be required to upload the associated 9-1-1 call audio. Submissions should be complete and accurate to ensure proper review and consideration.
